• Your Insurance Card

• Your Drivers License

• List of all current medications ( both prescription and OTC)

• List of questions you may wish to discuss with the doctor about your condition and/or treatment

• Previous medical records

• All previous radiology imaging reports and images (X-ray, MRI, CT-scan, Ultrasound)
